The Nigeria Data Protection Act 2023 gives you rights over your personal data. This page explains what those rights are and how to make a request to Lagos State Ministry of Health.

Your rights

  • Be informed about how we use your data.
  • Access the personal data we hold about you.
  • Ask us to correct inaccurate or incomplete data.
  • Ask us to delete data, where the law allows.
  • Ask us to restrict processing, where the law allows.
  • Object to processing, where the law allows.
  • Withdraw consent where processing relies on it.
  • Lodge a complaint with the Nigeria Data Protection Commission.

How to make a request

You can submit a request in either of these ways:

  • Use the contact form and choose the option that best describes your request, or say that it is a data-rights request in your message.
  • Email hello@healthradar.health with the subject line "Data rights request".

If you are the parent or guardian of a child under monitoring, you can make a request on their behalf.

What to include

  • Your full name and the phone number or email address used during monitoring.
  • The right you want to exercise.
  • Enough detail for us to find your record, such as the approximate arrival date and port of entry.
  • How you would like us to reply.

What happens next

We acknowledge your request, confirm your identity where needed, and respond within the period required by the NDPA 2023 and NDPC guidance. If a request is complex, we may need more time, and we will tell you why.

Where a request is refused, we explain the reason and tell you how to complain.

Limits on these rights

Public-health and record-keeping obligations can limit some rights. For example, we may need to keep a surveillance record for the monitoring window and any period required by law, even if you ask us to delete it. We will tell you when a limit applies and why.

Complaints

If you are not satisfied with how we handled your request, you can complain to the Nigeria Data Protection Commission. You can also contact us first so we can try to put it right.
